2008-2009 Informed Consent for Nursing Services Document Must Be Filled Out and on File at ICS

Attention ICS parents/guardians:

All students who wish to receive any nursing services at school must have a copy of the informed consent document signed, dated, and on file at the start of each school year. You may make a copy to turn in on your child's first day of school (see link below). Copies will also be available at school in the nurse's office and at the main office front desk. On Monday, July 28, the school nurse will be available in the gym from 1 p.m. - 7 p.m. to answer questions, provide needed medication consent forms, etc. Thank you for your cooperation!
